Health Management

A disease management program can help your employees manage chronic health conditions, including heart disease, diabetes and chronic respiratory conditions. We offer an easy-to-use disease management program that can help your employees lead healthier lives and may improve your bottom line.

Millions of people in the United States suffer from chronic conditions. Living with a chronic health condition can be costly — both to your employees and to your company. Chronic conditions cannot be managed by simply visiting the doctor a few times each year. Patients live with their conditions 24 hours a day, seven days a week, often administering nearly all of their care themselves. If your employees aren’t efficient self-managers, they can frequently be absent from work due to sick days or multiple doctor visits and hospital stays.

According to an article in Medical News Today, doctors say that education and self-management training are mainstays of most chronic diseases like asthma, diabetes and hypertension.1 With good self-management skills and an understanding of their doctors’ care plans, people with chronic conditions can better control their symptoms and are less likely to require hospital stays or emergency room visits. This could also result in lower health care costs, reduced absenteeism and increased productivity for accounts.

Health Management targets members with heart disease, diabetes and chronic respiratory conditions such as asthma. The program is designed to help your employees learn more about their conditions and become better self-managers. By offering a disease management program, you can reach out to individuals at risk and empower them with the knowledge, resources and support to manage their chronic conditions.

By offering health management, the end results for your company can include:

  • A healthier workforce
  • Reduced absenteeism
  • Fewer hospital stays
  • Improved worker satisfaction and retention

The Program

Health management identifies and targets not only existing high-cost members, but also potential at-risk members before they incur unnecessary health care costs. BlueCross delivers a patient-centered, proactive program of care that uses the latest management technology. To identify members with chronic conditions, we monitor laboratory, medical and pharmacy claims to identify patterns. Members identified as having heart disease, diabetes or chronic respiratory conditions will be enrolled in the Health Management program automatically. Participants receive materials to improve their self-management skills and ability to work with their doctors to take better care of themselves.

In addition to information and tools to help make positive behavior changes, participants have access to dedicated health coaches —  registered nurses who helps them learn more about their conditions and ways to manage them. By phone or through our online Personal Health Record, health coaches offer support to help participants work with their physicians and develop a self-care plan to take charge of their illnesses. Participants can contact their health coaches as often as they need for support.

The Personal Health Record is a safe and secure Internet-based health record available to members through the My Health Toolkit® feature of our website. With the Personal Health Record, members can access their consolidated health information 24 hours a day, seven days a week. They can track appointments, manage physician contact information, browse health articles and much more.

When enrolled in Health Management, a participant’s health coach can also send tasks and additional information regarding his or her conditions online. Participants can opt out of the program by simply calling their health coaches and declining participation.
